1. Register, Review Athletic Handbook, and NFHS Concussion Sign Off Online (CLOSED as of 2/28)
At the completion of the athletic registration form, you will be able to:
a) Review and sign Athletic Handbook
b) Review and sign NFHS Concussion Sign off Sheet
c) Please watch "NFHS Concussion in Sports" video prior to completeing this step (information to view this video listed below)
2. Pay User Fee
a) Bank check or money order, No Cash or Personal Checks Accepted
b) Made payable to the 'Town of Winthrop'
c) To be dropped off at WMS Main Office Attn: Mr. Mallios
3. Physical Form
a) Email or pass in a valid physical (valid for 13 months)
b) Regardless if the nurse has a copy, one must be turned into Mr. Mallios.
A COPY OF A VALID PHYSICAL AND USER FEE MUST BE RETURNED TO MR. MALLIOS PRIOR TO THE START OF THE SEASON
